MEDICAL
The signed parent liability release on the application as well
as a signed physical examination form is required for all campers. School
physicals from the 2006-07 school year dated after June 8, 2006 are acceptable.
Due to our increased liability and the insistence of our insurance carrier,
campers without proper forms will not be allowed to participate.
CONFIRMATION
Notice on the registration form there is a line for an email
address. We will send camp confirmation information via email this year.
If there isn’t a reliable email address, or you would prefer to
receive it by physical mail you may leave this item blank. However, email
is a quick source of communication for bulk emails in the case of emergency
changes or for asking questions.
ATHLETIC TRAINER
A Certified Athletic Trainer will be on duty at all camp sessions.
We recommend that players be in excellent physical condition to better
enable them to have the most positive learning experience as well as preventing
